New Patients at Duluth Injury & Rehab Center
We look forward to welcoming you to Duluth Injury & Rehab Center!
ONLINE FORMS
High Tech Check-In
We use HIPAA-compliant software that enables you to complete all of your paperwork in a paperless format, and allows us to maintain clean and concise documentation that we can submit to attorney offices, insurance companies and other health care facilities in a timely and efficient manner. It also helps the environment!
Upon entering our office, you will be provided a clipboard with a “welcome sheet” to fill out. That information will be used to set up your file in our system, and you will be assigned a patient number. After that, our friendly staff will help you complete the rest of your information on a tablet PC.
Your First Visit
Dr. Joe will then sit down with you for an in-depth consultation, discussing your current and past health history and other information relevant to your condition. Dr. Joe will perform a detailed examination consisting of palpation, range of motion, muscle test, orthopedic test, neurological test, and anything else that may be helpful in helping him to obtain an accurate diagnosis.
If X-Rays, an MRI or CT Scan are needed, we will refer you out for those. If Dr. Joe feels that you would benefit from medical co-management, we would also refer you to the appropriate professional. Our office works in a cooperative fashion with other healthcare professionals, including orthopedists, neurologists and pain management clinics.
After the exam, most patients receive appropriate care at this first visit. This may include electrical muscle stimulation, traction, therapeutic ultrasound, manual therapy techniques, cold laser therapy and/or various forms of spinal adjustments. All care is determined by Dr. Joe on a case by case basis.
Home Instructions
Prior to leaving our office, you will be given home care instructions. This may include the use of ice or heat, exercises and/or stretches, proper lifting techniques, nutritional advice and certain activities to avoid in order to prevent an exacerbation of your symptoms.
